- - Applicant must be a citizen of Nigeria by birth, or birth from either of the parents who must be a Nigerian.
- - Submission of filled and signed online passport form by the applicant.
- - Copy of old, and/or expired Nigerian Passport to be attached to the application.
- - Copy of marriage certificate or name change (addition or subtraction) supported by court affidavits should be attached to the application.
- - Applicant who are minors under sixteen (16) years of age, must submit/attach to the application, apart from the general requirements, the following:

- Letter of consent signed jointly or separately by both parents;
- Copies of data pages of parents’ passports;
- Signatures on passport data pages of both parents must match with their letters of consent;
- Birth Certificate of Child/Minor. Please note, single mothers must obtain court affidavit/judgment indicating full custody of child (copy should be attached) to the application.
- - Applicants above sixteen (16) years must apply and submit either of the parents (Nigerian) whose name must be in the Child’s Birth Certificate bearing the same last (surname) and data page of passport of either of the parents must be attached to the letter of consent/introduction.
- - Copy of payment confirmed slip printed online after payment of sixty five dollars ($65.00) passport fee.
- - Attach to the application thirty dollars ($30.00) money order as administrative/processing fee payable to Consulate General of Nigeria, New York should be submitted on arrival at the Consulate.
